GP Appointments
We currently have 7 GP's at our practice who offer face to face and telephone consultations. We have an appointment system called Total Triage. You can raise a triage request by completing a triage form using Accurx via our website or if you are unable to use this by phoning the practice. A member of our team will help you complete the Triage request by completing the questionnaire on your behalf. This usually takes less than 5 minutes.
The GP will assess your questionnaire and provide an appointment; prescription; or self care information.
You will be offered a solution within 24 - 48 hours of your request depending on the clinical urgency of your request. This means that if your need is urgent, based on the GP's advice, you will be offered a same day appointment, or a next day appointment should you contact us later in the day. Routine requests are either 5 - 7 day or 14 - 21 day appointment, based on the GP's advice.
Musculoskeletal
Musculoskeletal problems are seen by our in house first contact practitioners. We have 3 in total whose sessions are either on a Tuesday or a Thursday. They specialise in musculoskeletal issues and they are able to refer you on to secondary care. These appointments are bookable after completion of the Total Triage request and GP's guidance.
Nurse Appointments
If you have been asked to book an appointment through a text message, you can follow the link attached and book directly into our appointment system, without having to speak to a receptionist. If you have any queries, please call reception.
Pharmacists
Our clinical pharmacist are able to offer medical reviews and deliver minor ailment clinics. The medical reviews can be either face to face or a telephone consultation. These are available to book online through patient access. Both pharmacists can perform Pill Reviews. If you are due for a pill review, we will invite you using a self book link. This link will allow you to book in with the pharmacists for all available appointments.
Text Reminder Service
We have a texting service which allows you to receive confirmation and reminders about your appointments.
This is set up so you will receive a reminder 7 days and 24 hours before an appointment. It will also send you a link so that you are able to cancel your appointment automatically, if necessary.
To have this service you will need to register by completing a consent form.

Enhanced Access
We are pleased to be able to offer our patients access to routine appointments in the evenings and weekends.
Primary Care Networks (PCNs) provide enhanced access appointments between the hours of 6.30 pm to 8 pm Mondays to Fridays and 9 am to 5 pm on Saturdays. As enhanced access will be delivered at a PCN level – not every practice has to be open during these hours.
We are pleased to be able to offer our patients, as part of Trent Care Network PCN, access to routine, pre-booked appointments at evenings and weekends. These are referred to as Enhanced Access appointments. The service is being delivered at the Cleveland Surgery, Gainsborough on behalf of the PCN and is available for any patient who is registered at any of the practices below:
- Caskgate Street Surgery
- Cleveland Surgery
- Hibaldstow Medical Practice
- The Glebe Practice
- Trent Valley Surgery
Enhanced Access hours are between 6.30pm-8pm weekdays and 9am-5pm on Saturdays.
Appointments are for pre-bookable, non-urgent consultations (e.g., chronic illness, asthma checks, and blood pressure checks) with GPs, Nurses and Health Care Assistants. Please note, the enhanced access service is not a walk-in service.
As your registered practice (including Cleveland Surgery) will not be physically open during these hours, patients will be unable to have general queries answered or dealt with during enhanced access opening times. Please be aware, your appointment may be a telephone call, video call or a face-to-face appointment, you will be advised at the time of booking.
When booking an appointment, you will need to consent to a Consulting Clinician that may not be your own GP viewing and updating your health record.

For urgent matters at evenings, weekends and bank holidays, patients will continue to access the GP Out of Hours Services at Lincoln County Hospital via NHS 111.
Interpreter
If you require an interpreter to attend with you when you see your Doctor please notify the receptionist and she will arrange this for you.
Chaperone
If you require a chaperone during your appointment please let us know and we can arrange for someone to accompany you during an examination or private discussion.
